Specialized Programs:

Senior Restoration Program
The objective of our Senior Restoration Program is to assist patients to achieve a higher level of functionality in order to continue to live independently at home. The program focuses on physical activation and psychological rehabilitation. It is designed specifically for the senior with physical and/or mental impairments due to depression, dementia, cardiovascular disease, musculoskeletal pain and/or diabetes. First, we will screen and evaluate the patient’s risk factors based on their baseline health status and design an individual plan around personal goals. Second, we start a comprehensive exam and medical file review to ensure a proper diagnosis has been made. Finally, we utilize cost-effective clinical management for each patient and recommend or implement a course of treatment. Once the patient reaches a level of health assuredness, compliance and physical/mental stability, the treatment then becomes focused on active rehabilitation.

Criteria for Inclusion:

Candidates for this program are 55 years of age or older and have a mild to moderate mental or physical impairment. Our clinical team will screen the patient prior to admission to the program. Our Nurse Case Coordinator oversees treatment from admission to discharge.

Course of Treatment:
Treatment consists of a 2-day per week, 10-16-week program. Length of the program will depend upon the client’s severity of impairments and treatment goals. Components of our program will be on-site and some portions may be provided in home.
